USMLE: United States Medical Licensing Examination

The United States Medical Licensing Examination is a three step examination for medical licensure in the United States. The license provided by ECFMG requires USMLE Step 1, Step 2 CK and Step 2 CS. Step 3 is not required for licensure or the US residency match but must be completed within 7 years of taking your first USMLE step otherwise the steps will need to be repeated. USMLE Step 1, Step 2 CK and Step 2 CS can be taken in any order.

Eligibility
You must be a medical student officially enrolled in, or a graduate of, a medical school listed in the World Directory of Medical Schools as meeting ECFMG eligibility requirements. To register for ECFMG certification and apply for any exams, a $65 fee is paid and an ECFMG ID number must be received. To receive ECFMG certification, you must be a graduate and provide your certified graduation certificate. You are allowed to take your exams before graduating, but will not receive your ECFMG certificate to practice medicine or participate in a US residency program until you graduate. You will also require ECFMG certificate to take Step 3.
 

Step 1
Step 1 is a one-day computer based MCQ examination that includes 308 multiple-choice items divided into 7 blocks of 44 items; 60 minutes are allotted for completion of each block of test items. On the test day, examinees have a minimum of 45 minutes of break time and a 15- minute optional tutorial. This exam can be taken at any Prometric center worldwide, including Amideast which is located in Cairo. A 3 month eligibility period is chosen upon applying for the exam, during which the student will be allowed to register for the date that they want. The exam is available on many days each month. The fees are $1045. Step 1 assesses whether you understand and can apply important concepts of the basic sciences necessary for the practice of medicine, with special emphasis on principles and mechanisms underlying health, disease, and modes of therapy. These basic sciences include anatomy, physiology, biochemistry, pathology, microbiology, pharmacology, behavior sciences, biostatistics, epidemiology as well as interdisciplinary topics.

Step 2 CK
Step 2 CK is a one-day computer based MCQ examination that includes 8 blocks of multiple-choice items that will not exceed 45 questions per block or a total of 355 questions; 60 minutes are allotted for completion of each block of test items. On the test day, examinees have a minimum of 45 minutes of break time and a 15- minute optional tutorial. This exam can be taken at any Prometric center worldwide, including Amideast which located in Cairo. A 3 month eligibility period is chosen upon applying for the exam, during which the student will be allowed to register for the date that they want. The exam is available on many days each month. The fees are $1065. Step 2 CK assesses whether you can apply medical knowledge, skills, and understanding of clinical science essential for the provision of patient care under supervision and includes emphasis on health promotion and disease prevention. Step 2 CK topics include applying foundation science concepts, patient diagnosis, patient management, medicolegal issues, patient safety, biostatistics and epidemiology in all clinical specialties.

Step 2 CS
Step 2 CS is a one-day clinical exam which will include twelve patient encounters, a few of which are not scored. You will have 15 minutes for each station followed by 10 minutes to type a patient report. This exam must be taken in one of 5 testing centers located in the US; Houston, Chicago, Atlanta, Philadelphia or Los Angeles. After registering for the exam, the student is awarded a 12 month eligibility period during which they can pick any suitable date at any test center. Exams are held almost daily, but early placement should be considered to get a suitable spot due to the high number of applicants in particular centers. The fees are $1550. Step 2 CS uses standardized patients to test medical students and graduates on their performance in 3 subcategories: Communication and Interpersonal Skills (CIS), Spoken English Proficiency (SEP), and Integrated Clinical Encounter (ICE). Step 2 CS assesses the students’ ability to gather information from patients, perform physical examinations, and communicate their findings to patients and colleagues.
 

Step 3
Step 3 is a two-day exam. Day 1 tests for Foundation of Independent Practice (FIP) through 6 blocks of 42 to 43 MCQ items; 60 minutes are allotted for completion of each block of test items. Day 2 assesses Advanced Clinical Medicine through 6 blocks of 33 MCQ items with 45 minutes available for each block, as well as 13 computer based case simulations with 10-20 minutes of time allowed for each case. After registering for the exam, the student is awarded a 6 month eligibility period during which they can pick any suitable date at any test center in the US. Exams are held several times a month. The fees are $830. Topics include applying foundation science concepts, patient diagnosis, patient management, medicolegal issues, patient safety, biostatistics and epidemiology in all clinical specialties and in a variety of clinical settings. Step 3 can only be taken in a US based Prometric center after completing Step 1, Step 2 CK, Step 2 CS and receiving ECFMG certification. Step 3 is not necessary for ECFMG certification or applying for the US residency match, but it must be completed within a 7-year period from your first USMLE exam.
